Registered Nurse (RN) Case Manager – Home Health
Location: St. Cloud, MN | Pay: $44–$46/hour | Type: Full-time | Schedule: Monday–Friday, rotating weekends, on-call
Overview:
Seeking a dedicated RN Case Manager to coordinate and deliver in-home nursing care. This role involves managing a caseload of approximately 45 clients, performing clinical assessments, care planning, and acting as a liaison among care teams. Includes field nursing tasks such as wound care, glucose checks, and medication setup.

Minimum Requirements:
- Valid and unrestricted Nurse (RN) license in the State of Minnesota
- BLS certification (required)
- Strong Clinical skills and the ability to conduct comprehensive assessments
- Minimum one year of nursing experience (home health or hospice preferred)
- Exceptional communication and interpersonal abilities
Key Responsibilities:
- Perform comprehensive assessments and maintain individualized care plans
- Coordinate with providers and supervise unlicensed staff
- Complete reauthorizations and maintain accurate documentation
- Provide health education and support to clients and families
- Deliver hands-on nursing care (vitals, MARS/TARS, discharge planning, etc.)

Compensation: $44.00 - $46.00 per hour
